In the event that a child’s parents are divorced, we will require consent from both parents for evaluation and/or treatment. We require custody decrees on file before scheduling.
Missed Appointments/Cancellations
Your appointment time is reserved exclusively for you. If you need to cancel or reschedule your appointment for any reason, please make sure to do this at least 24 hours in advance. A $560 fee will be assessed for last minute cancellations and missed appointments and $100 fee for in office testing. Although we realize that life does not always go as planned, those who no-show to appointments consistently will be referred elsewhere.
